We neglected to cite the source of two quotes from a
presentation to a client.

A member of our team discovered the error.

Anyone who was present at our training session.
How has the Error been corrected?
Yes, we have corrected the error.
Democratic National Convention Speech by the late
Honorable Barbara Jordan of Texas (Feb 2006)
What is the Error?
We mistakenly quoted former Gov. Anne Richards of Texas as
the origin of the famous speech on "from what . . . to what" at the
1992 Democratic Convention.
Where does the Error occur?
In our speaking materials on the topic of change management.
Who reported the Error?
A client pointed out our error during one of our talks on the topic.
Who was impacted by the Error?
Anyone who was present at our discussion and heard our response
incorrectly reference the quote from this speech .
How has the Error been corrected?
Yes, this error occurred in our discussion points, not in our
written materials. We have corrected the error.
Respondable Risk (May 2005)
What is the Error?
We
mistakenly indicated a common trademark (™)
on "Respondable Risk," a term we have no intention of
registering with the United States Patent and Trademark Office.
Where does the Error occur?
In our print materials and correspondences that we
shared with clients, potential clients and friends of the
Institute..
Who reported the Error?
A member of the Institute team discovered the error
while reviewing our materials.
Who was impacted by the Error?
Anyone who has viewed the materials marked with this
mistake, and formed the impression that we intended to
register "Respondable Risk" with the USPTO.
Strictly speaking, no one has been negatively impacted by this
mistake as we present no claims to the term.
How has the Error been corrected?
We have discontinued use of any materials in our
possession with the erroneous trademark (™)
symbol associated with "Respondable Risk." But
some of our pamphlets and letters in distribution still contain the
error.
